This short questionnaire is designed to help you clarify your desired direction in life and recognize what you want to get out of coaching. These questions intend to help you pinpoint areas of improvement in your life. You can fill out as much or little as you like. A coach can work on all these areas with you and help you move forward.

Your Wheel Of Life

Take 5 minutes to fill out your wheel of life. The aim of this exercise is to raise your awareness to how satisfied or fulfilled you feel in different areas of your life at any one moment.

Rate how satisfied you feel in each area of your life from one to ten, 1 being very dissatisfied and 10 being very satisfied. Once you have finished, join up each area and see how smooth your wheel is (it may be easier to print out slide # 2). Be instinctive, you are looking for an impression of how you feel right now, not a logical, rational debate with yourself.
